The following forms and fees must be completed on your parent portal:
ESHS Re-Registration Contract – This form must be completed by all families who have a student attending Elizabeth Seton during the 2022-2023 school year. If the student has two agreements in FACTS for tuition and fee payments, we must have two completed and signed re-registration contracts. The completion of this form will require an online payment of the $200 re-registration fee and, if a rising senior, the payment of the $200 graduation fee.
Household Information Verification Form – This form requires you to review and update the household information we have in our database for your daughter. ONLY complete the Student Sibling section of the form if you have multiple daughters at Seton or sons attending DeMatha during the 2022-2023 school year. Please sign and submit when completed.
Families will automatically be re-enrolled in FACTS for the next school year. An annual enrollment fee of $50.00 will be charged to you by FACTS prior to the start of tuition payments for 10 month plans and an annual enrollment fee of $20 will be charged for one and two pay tuition plans. The first payment is due June 5th or 20th for 10 month plans.
FACTS Agreements for the 2022-2023 school year will be finalized in May and each family will receive an email notification of the upcoming year’s tuition agreement sent to the email listed on file for your FACTS account.
Reminder: We cannot build any student’s schedule without re-registration information. Students who complete re-registration late may not be able to get in the courses they want for next year.
